VAR Change Amendment - Use this form
to notify Medisoft that you are changing to using Frontier as your Value Added
Reseller.
Electronic Claims Carrier Agreements - Forms required by insurance carriers
for submitting claims electronically through Medavant (the Per-se Technologies
clearinghouse).
Form 855B -
Enrollment form for Clinics, Group Practices, and Incorporated sole
practitioners. This form can be filled-in online using Adobe Acrobat, then printed, or you
can print it out and complete it by hand.
Form 855I -
Enrollment form for Physicians and Non-Physician Practitioners. This
form can be filled-in online using Adobe Acrobat, then printed, or you can print
it out and complete it by hand.
Form 588 - EFT
Authorization. Agreement to have funds electronically transferred by
Medicare to your bank account.
